Article 14. Monthly reporting🔗

1. Monthly reporting shall be carried out by the district public health centres. The monthly report form IV-03/1 contains aggregated numerical data on both the notifiable diseases subject to urgent notification and the monthly notification for certain notifiable diseases (e.g. influenza, g.r.c. infections, amoebiasis, etc.). Data aggregation shall be carried out from the monthly notification form 58/3 submitted by medical institutions. A working book shall be used for data aggregation.

2. All cases whose infection occurred in the territory of a given district shall be included in that district's report. Accordingly, a case that was only detected in another district shall not be included in the report of the district of detection.

3. Monthly reports shall be prepared in accordance with the form presented below and sent to the regional public health centres in 2 copies no later than the 5th day of the following month.

4. In the event that not even a single case of a specific disease has been registered by a medical institution during the given reporting period, the corresponding column for the relevant disease under "Total" shall indicate "0" (instead of leaving an empty space). This distinguishes "incompletely recorded cases" from "absence of cases".

5. A group of nosologies, for example "other bacterial intestinal infections" – A04, unites various diseases. If during the reporting period there were cases of esherichiosis and yersiniosis, as well as other bacterial intestinal infection united by A04, then in the corresponding column – other bacterial intestinal infections – their total number shall be indicated.

6. In the statistical forms of the monthly report, cases that have not yet been finally classified shall be indicated. The diagnosis may change after receiving additional laboratory or epidemiological information. In such a case, the reports of the previous month shall be corrected.

7. Together with the monthly report form, the district public health centre shall send to the regional public health centre, in 2 copies, the case/outbreak epidemiological investigation reports in cases of the specified diseases/conditions subject to urgent notification, and the anti-rabies work report in the standard form.

8. One copy of every statistical report shall remain on site.

9. Of the 2 copies of the monthly reports received from the district public health centres, one shall be sent to the DKSSEC. Along with this, the information received through the report forms shall be entered into the electronic database, an aggregated report for the entire region shall be prepared (with the same structure) and sent to the DKSSEC by the 7th day of the following month.

Article 15. Annual reporting🔗

1. Medical institutions shall not be required to send annual reports. The regional and district public health centres shall report according to form IV-03 through the same procedures indicated in the monthly reporting section (data shall be summarised from the monthly notification submitted by medical institutions. Working books are provided for data summarisation).

2. Rules for completing the monthly summary notification (form N58/3) and the reporting forms (monthly, annual)

a) columns "3-9" – indicate the number of cases recorded in the reporting month/year by age groups;

b) column "10" – indicate the total number of cases, which shall coincide with the sum of the numbers indicated in columns 3-9;

c) column "11" – indicate only the number of cases that are tested in a laboratory;

d) column "12" – indicate only the number of cases that are confirmed in a laboratory;

e) column "13" – indicate the total number of "confirmed" cases (laboratory or epidemiological link), which corresponds to the final classification of cases in accordance with the standard definitions.

Chapter V. Data analysis

Article 16. General provisions🔗

1. The purpose of data analysis is:

a) identification of the causes giving rise to problems;

b) finding the most adequate ways of solving problems;

c) identification of trends;

d) assessment of the quality of disease prevention and control measures/programmes

e) for interim and long-term periods;

f) forecasting of the epidemiological situation for a short-term or long-term period;

g) determination of high-risk areas;

h) identification of high-risk contingents.

2. The essence and scope of the analysis is determined by:

a) the level of the public health system (where the analysis is carried out);

b) the nature of conducting the analysis: routine (monthly or annual) or emergency (e.g. during an outbreak).

3. A high-quality analysis requires the existence of complete and accurate epidemiological surveillance data.

4. Data analysis is usually carried out by time (e.g. multi-year or monthly trend), place (e.g. subordinate district or institution), demographic and medico-biological characteristics (e.g. age group, sex, immunisation status).

5. Data analysis is carried out at all levels of public health, with varying scope and periodicity.

Article 17. Emergency analysis🔗

1. Emergency analysis (during disease outbreaks) is carried out on an extraordinary basis by the SSC upon detection of cases that are subject to emergency notification by the district SSC at the regional and central levels (see Table 1. Diseases subject to emergency notification and notification rules) and includes: determination of the causes of fatal outcome (if such occurred), grouping of diseases and fatal cases (taking into account the characteristics of the diseases/conditions): by date of onset, territorial distribution (usually represented on a map, in a table or by histogram), by age groups, sex, immunisation status, organisation (workplace, educational institution, kindergarten), the level of attendance therein and other known risk factors.

2. The emergency analysis is drawn up in written form, kept on site for 5 calendar years and submitted to the superior requesting organisation in accordance with the applicable legislation and the request.

3. The table below presents the list of types of emergency analysis for each level:

3. Routine analysis is drawn up in written form, kept on site for 5 calendar years and submitted to the superior requesting organisation in accordance with the applicable legislation and the request.

4. Diseases targeted for elimination (e.g. poliomyelitis) require routine analysis of additional information determined by the WHO. As a rule, such analysis relies on the epidemiological investigation of each case, is carried out by national experts and requires the provision of additional information to the NCDC in accordance with the request.

Article 19. Methods of analysis (causes of death)🔗

1. Ensures that the public health services carry out appropriate measures in the future for the purpose of preventing fatal cases. The analysis of causes of death caused by infectious diseases includes the review of information about cases (medical history, notification, records of journals 60/A and 60/B) and the prevention of the following possible cases.

Causes of death

Public health measures

1. The patient sought medical attention at a medical institution too late

Raising the level of awareness of society/the population in matters of health and medical services, explaining the harm of self-treatment and rejecting it

2. The diagnosis was made in a timely manner, but treatment was not carried out

Ensuring compliance with standard disease management schemes,

taking measures against "unofficial" treatment

3. The diagnosis was made in a timely manner, but treatment was delayed

(medicines are unavailable and their timely supply did not occur)

Proper provision of medicines,

informing practitioners that improper communication and cooperation may become the cause of a patient's death

4. Inadequate treatment was carried out (due to an incorrect diagnosis or other reasons)

Improving the knowledge of medical personnel

5. There was resistance to medicines

Updating standard case treatment schemes

6. Ineffective vaccination

Determination of vaccination effectiveness, assessment of compliance with cold chain and vaccination rules

Article 20. Methods of Analysis (Case Fatality Rate)🔗

1. It is an indicator of the severity of a disease and at the same time indirectly indicates the adequacy of case detection and management.

2. Currently in Georgia, due to the improvement of disease control, cases of fatality associated with infectious diseases are few. Accordingly, determining the said indicator is meaningful only at the regional or country level.

3. An increase in the case fatality rate is the basis for reviewing the accessibility of healthcare services, timely treatment, and standard treatment regimens.

Article 21. Methods of Analysis (Morbidity/Incidence Trends)🔗

1. Routine monitoring of morbidity shall be carried out in all public health centres and in those medical institutions that serve 5,000 or more people.

2. Monitoring is usually carried out by the physician(s) who are directly involved in the process of disease detection and treatment, by epidemiologists (NCDC) or statisticians. The data are regularly checked by the chief physician/head of the institution and, for the purpose of decision-making, transmitted in any form to the healthcare heads of the given territory and to other persons interested in the said information in accordance with the existing legislation.

Article 22. Methods of Analysis (Breakdown of Disease/Fatal Cases/Indicators by Territory)🔗

1. Analysis of data by territory provides information on the location of disease cases and makes it possible to determine the causes of the spread of infection (if such occurred). Routine analysis by territory is usually carried out by the NCDC, however, during disease outbreaks it is recommended that individual healthcare institutions carry out such analysis using local maps and mark the places that may be important for the given disease (e.g., schools, markets, water supply systems).

Article 23. Methods of Analysis (Breakdown of Disease/Fatal Cases/Indicators by Age Groups and Immunization Status)🔗

1. Such analysis determines the high-risk group and identifies certain risk factors.

2. With this information, the manager determines the form of response (e.g., improvement of vaccination coverage or preventive educational activity).

Article 24. Methods of Analysis (Morbidity/Incidence by Age-Sex-Immunization Status-Organized Setting and Other Specific Indicators)🔗

1. Incidence enables public health workers to:

a) carry out monitoring of a disease over a defined period of time in different territories;

b) determine groups of people at high risk and identify potential risk factors for the given disease (in this case, sex, age and other characteristics important for the given population group).

2. For a more detailed and clear characterization of the epidemic process, it is sometimes necessary to use more refined indicators, which make it possible to determine specifically which group of the population was affected. The population may be divided into groups by: age, sex, occupation, organized setting, immunization status, and others.

3. Determination of specific morbidity indicators by age, occupation and other factors is possible only if reliable statistical data on the denominator (the size of the specific population group) are available.

Article 25. Methods of Analysis (Case Confirmation Indicators, Proportion of Laboratory-Confirmed Cases)🔗

1. In an effectively functioning epidemiological surveillance system, the disease confirmation rate is higher than 70-80%. For diseases in the elimination phase (polio, measles), this indicator will presumably reach 100%.

2. According to case definitions/case classification, a case is considered confirmed if it is confirmed by means of a laboratory test specific for the given disease and/or an epidemiological link of this case to another laboratory-confirmed case has been established.

3. The proportion of lab-tested and lab-confirmed cases is determined separately. Their comparison (the proportion of confirmation of lab samples) makes it possible to identify improperly functioning medical institutions/laboratories or districts, the potential causes of unsatisfactory performance, and to plan measures to remedy the situation.

Article 26. Analysis methods (vaccine effectiveness)🔗

1. The term "vaccine effectiveness" describes the ability of a vaccine to prevent a disease after vaccination.

2. The goal of vaccination – to prevent a disease – depends on its effectiveness and the proper management of the immunisation process. Since the effectiveness of vaccines is usually below 100% (in some people seroconversion does not occur at the initial stage, while in others immunity wanes rapidly), there will always be cases of the disease among the vaccinated. Field evaluations of vaccine effectiveness are very important, especially when vaccine effectiveness is called into question due to the spread of the disease among the vaccinated.

3. Monitoring this indicator allows for the determination of the long- and short-term prospects of immunisation.

Article 27. Analysis methods (timeliness and completeness/accuracy of reporting)🔗

A report is considered timely if it is received within the deadline established by the higher-level institution;

1. A report is accurate if it is filled out completely, the next-level institution has received explanations in accordance with the request (if necessary), and during verification by comparison with the institution's own records no discrepancy has been identified (e.g., urgent notification, results of case investigation, etc.).

Article 28. Analysis methods (case/outbreak investigation indicator)🔗

Case/outbreak investigation is conducted in accordance with the requirements of Table 1 (requirements for notification, reporting and epidemiological investigation by nosologies), which shall be updated as necessary by the NCDC.

1. If the investigation indicator is low (less than 90%-100%), the causes must be identified and corrective measures taken.

2. Such analysis must be conducted by the district PHS for the purpose of self-assessment.
