ბ) ჟურნალის წარმოებაზე პასუხისმგებელია მასწავლებელი, ხოლო მასწავლებლის მიერ მოსწავლეთა გაკვეთილზე დასწრების ყოველდღიური აღრიცხვის წარმოებაზე – კლასის დამრიგებელი. სკოლის დირექტორი ვალდებულია განსაზღვროს უფლებამოსილი პირი, რომელიც მონიტორინგს გაუწევს კლასის დამრიგებელთა მიერ მოსწავლეების გაკვეთილებზე დასწრების აღრიცხვის კონტროლს. ასეთი უფლებამოსილი პირი შესაძლოა იყოს სკოლის დირექტორის მოადგილე; გ) ჟურნალის არასწორი წარმოება, მისი დაკარგვა/გაფუჭება, ფურცლის ამოხევა და ა. შ.  წარმოადგენს დისციპლინურ გადაცდომას და სკოლის შინაგანაწესით განსაზღვრული დისციპლინური სახდელის გამოყენების საფუძველს; დ) დაუშვებელია ჟურნალის გატანა სკოლის ტერიტორიიდან, გარდა კანონმდებლობით გათვალისწინებული შემთხვევებისა; ე) კლასის დამრიგებელი ვალდებულია ყოველთვიურად განსაზღვროს მოსწავლის საპატიო და არასაპატიო გაცდენები და უზრუნველყოს აღნიშნულის გამო მოსალოდნელი შედეგების თაობაზე მოსწავლის მშობლის დროული ინფორმირება;  ვ) კლასის დამრიგებელი ვალდებულია სასწავლო წლის დაწყებისთანავე უზრუნველყოს მოსწავლისა და მისი მშობლის ინფორმირება მოსწავლის მიერ გაკვეთილების გაცდენის შედეგად მოსალოდნელი სავარაუდო შედეგების შესახებ. 2. ჟურნალის წარმოება ა) ჟურნალის ყოველი გვერდი უნდა დაინომროს და დამოწმდეს სკოლის ბეჭდით; ბ) ჟურნალის გარეკანის შიდა მხარეს ხელი უნდა მოაწეროს  სკოლის დირექტორმა შესაბამისი თარიღის  მითითებით და დასვას სკოლის ბეჭედი; გ) ჟურნალის წარმოება ხორციელდება კონკრეტული კლასისთვის ,,ეროვნული სასწავლო გეგმის დამტკიცების შესახებ” საქართველოს განათლებისა და მეცნიერების მინისტრის 2006 წლის 28 სექტემბრის №841 ბრძანებით დამტკიცებული ეროვნული სასწავლო გეგმით (შემდგომში – ეროვნული სასწავლო გეგმა) და სასკოლო სასწავლო გეგმით გათვალისწინებული საგნების მიხედვით; დ) გაკვეთილზე მოსწავლეთა დასწრების აღრიცხვა ხდება გაკვეთილის დაწყებისთანავე და გაკვეთილზე მოსწავლის არყოფნის შემთხვევაში ჟურნალში ფიქსირდება ,,არა“. ე) ჟურნალში დაუშვებელია გადასწორება, ნაწერის ამოშლა, კორექტორის გამოყენება და ა.შ. ჟურნალში დაშვებული შეცდომის გასწორება უნდა მოხდეს შესაბამისი გვერდის ბოლოს შეცდომის დაშვების თაობაზე მინიშნების გაკეთებით,  რასაც შეცდომის დამშვები პირი შენიშვნების გვერდზე სწორი მონაცემების ასახვით ადასტურებს ხელმოწერითა და თარიღის მითითებით; ვ) საკლასო განაყოფისა და არჩევითი საგნების მასწავლებლები ვალდებულნი არიან ინდივიდუალურად აწარმოონ გაკვეთილზე მოსწავლეთა დასწრების აღრიცხვა და მონაცემები დღის ბოლოს გადაიტანონ ჟურნალში. 3. მონაცემები, რომელსაც შეიცავს ჟურნალი ა) ჟურნალის გარეკანზე მიეთითება ჟურნალის სახელწოდება,  სკოლის სრული სახელწოდება, კლასი, რომლისთვისაც განკუთვნილია ჟურნალი, კლასის დამრიგებლის სახელი,  გვარი და შესაბამისი სასწავლო წელი; ბ) ჟურნალის გარეკანის შიდა მხარეს მიეთითება მოსწავლეთა სახელი და გვარი,  თითოეული მოსწავლის პირადი ნომერი და მოსწავლეთა რაოდენობა მიმდინარე სასწავლო წლის დასაწყისისთვის; გ)  ჟურნალის პირველ გვერდზე/გვერდებზე განთავსებული უნდა იყოს ჟურნალის წარმოების წესი; დ) ჟურნალში მოსწავლეთა მოძრაობის აღრიცხვის გვერდზე შეიტანება ინფორმაცია მოსწავლეთა სკოლაში მოძრაობის შესახებ, კერძოდ: დ.ა) მოსწავლის სახელი და გვარი; დ.ბ) მოსწავლის სკოლაში ჩარიცხვის თარიღი; დ.გ) იმ სკოლის სრული სახელწოდება, სადაც მოსწავლე სწავლობდა ამ სკოლაში ჩარიცხვამდე (ასეთის არსებობის შემთხვევაში); დ.დ) მოსწავლის სკოლაში ჩარიცხვის ბრძანების ნომერი; დ.ე) მოსწავლის სკოლიდან ამორიცხვის შემთხვევაში, სკოლიდან ამორიცხვის თარიღი; დ.ვ) სკოლა, რომელშიც გადავიდა მოსწავლე ამორიცხვის შემდეგ; დ.ზ) სკოლიდან ამორიცხვის ბრძანების ნომერი. ე) ჟურნალში სასკოლო სასწავლო გეგმით გათვალისწინებული საგნების შესახებ ინფორმაციის  ამსახველ გვერდზე შეიტანება შემდეგი მონაცემები: ე.ა) იმ საგნების სახელწოდება, შესაბამის მასწავლებელთა სახელისა და გვარის მითითებით, რომელთა სწავლება გათვალისწინებულია სასკოლო-სასწავლო გეგმით კონკრეტული სასწავლო წლისათვის; ე.ბ) მითითება ჟურნალის იმ გვერდების შესახებ, რომლებიც განკუთვნილია კონკრეტული საგნისათვის. ვ) ჟურნალში მოსწავლეთა დასწრების აღრიცხვის გვერდზე შეიტანება მონაცემები გაკვეთილზე მოსწავლეთა ყოველდღიური დასწრების შესახებ, სადაც სპეციალურად გამოყოფილ გრაფებში ასევე მიეთითება დღის განმავლობაში დაწერილი გაცდენების – „არების” რაოდენობა, გაკვეთილის თემატიკის/დავალების მოკლე სახელწოდება/შინაარსი და თემატიკის თარიღი; ზ) ჟურნალში მოსწავლეთა შესახებ ზოგადი მონაცემების გვერდზე აღინიშნება მოსწავლის საცხოვრებელი ადგილის მისამართი, მოსწავლის საკონტაქტო ტელეფონის ნომერი და მოსწავლის დაბადების თარიღი (რიცხვი, თვე, წელი); ბ) ჟურნალის ყოველი გვერდი უნდა დაინომროს და დამოწმდეს სკოლის ბეჭდით; გ) ჟურნალის გარეკანის შიდა მხარეს ხელი უნდა მოაწეროს  სკოლის დირექტორმა შესაბამისი თარიღის მითითებით და დასვას სკოლის ბეჭედი; დ) ჟურნალის წარმოება ხორციელდება კონკრეტული კლასისთვის ,,ეროვნული სასწავლო გეგმის დამტკიცების შესახებ” საქართველოს განათლებისა და მეცნიერების მინისტრის 2006 წლის 28 სექტემბრის №841 ბრძანებით დამტკიცებული ეროვნული სასწავლო გეგმით (შემდგომში – ეროვნული სასწავლო გეგმა) და სასკოლო სასწავლო გეგმით გათვალისწინებული საგნების მიხედვით; ბ) ჟურნალის გარეკანის შიდა მხარეს მიეთითება მოსწავლეთა სახელი და გვარი, თითოეული მოსწავლის პირადი ნომერი და მოსწავლეთა რაოდენობა მიმდინარე სასწავლო წლის დასაწყისისთვის; გ)  ჟურნალის პირველ გვერდზე/გვერდებზე განთავსებული უნდა იყოს ჟურნალის წარმოების წესი; ე.ბ) მითითება ჟურნალის იმ გვერდების შესახებ, რომლებიც განკუთვნილია კონკრეტული საგნისათვის. ვ) ჟურნალში საგნების მიხედვით შეფასების კომპონენტებისა და მათი ხვედრითი წილის შესახებ ინფორმაციის გვერდზე ასახულ უნდა იქნეს სასკოლო სასწავლო გეგმით გათვალისწინებული თითოეული საგნის შეფასებისათვის ეროვნული სასწავლო გეგმით დადგენილი კომპონენტებიდან 3 სავალდებულო კომპონენტის დასახელება. სკოლა უფლებამოსილია 3 სავალდებულო კომპონენტის გარდა შეფასების კომპონენტების ჩამონათვალში გაითვალისწინოს ეროვნული სასწავლო გეგმით განსაზღვრული სხვა კომპონენტიც.  კომპონენტების  ხვედრითი წილი ერთობლიობაში უნდა შეადგენდეს 100%-ს; ზ) ჟურნალში კონკრეტულ საგანში მოსწავლეთა აკადემიური მოსწრების/შეფასების შესახებ ინფორმაციის ამსახველ გვერდზე შეიტანება შემდეგი მონაცემები: ზ.ა) გაკვეთილის თემატიკის/დავალების მოკლე სახელწოდება/შინაარსი და თემატიკის თარიღი; ზ.ბ) მოსწავლის მიერ მიღებული მიმდინარე განმსაზღვრელი შეფასება კომპონენტების მიხედვით; ზ.გ) გაკვეთილზე მოსწავლეთა ყოველდღიური დასწრება სპეციალურად ამ მიზნისათვის გამოყოფილ მრავალკომპონენტიანი შეფასების წინ მოთავსებულ უჯრაში; ზ.დ) დღის განმავლობაში დაწერილი ,,არებისა“ და კომპონენტების მიხედვით ქულების ჯამური რაოდენობა გრაფაში ,,სულ“. თ) ჟურნალში გაცდენების აღრიცხვის გვერდზე კლასის დამრიგებლის მიერ შეიტანება შემდეგი მონაცემები: თ.ა) საგნის სახელწოდება; თ.ბ) საგნისათვის სასწავლო წლის განმავლობაში გათვალისწინებული საგაკვეთილო საათების რაოდენობა; თ.გ) სასწავლო წლის განმავლობაში საგანში გათვალისწინებული საგაკვეთილო საათების რაოდენობიდან გამომდინარე საპატიო და არასაპატიო გაცდენების ზღვრული რაოდენობა წლის განმავლობაში; თ.დ) ტრიმესტრის განმავლობაში ყოველი 4 კვირის მონაცემების მიხედვით  მოსწავლის გაცდენების საერთო რაოდენობა, მათ შორის, საპატიო და არასაპატიო გაცდენების რაოდენობა; თ.ე) სასწავლო წლის განმავლობაში გაცდენების საერთო რაოდენობა, მათ შორის, საპატიო და არასაპატიო გაცდენების რაოდენობა. ი) ჟურნალში ტრიმესტრული და წლიური ნიშნების ამსახველ გვერდზე შეიტანება მონაცემები სამივე ტრიმესტრის განმსაზღვრელი ნიშნის, სკოლის მიერ დანიშნული დამატებითი და საშემოდგომო გამოცდის, ექსტერნატის გამოცდის, საგნის წლიური ნიშნისა და საერთო წლიური ნიშნის შესახებ. კ) ჟურნალში მოსწავლეთა შესახებ ზოგადი მონაცემების გვერდზე აღინიშნება მოსწავლის საცხოვრებელი ადგილის მისამართი, მოსწავლის საკონტაქტო ტელეფონის ნომერი და მოსწავლის დაბადების თარიღი (რიცხვი, თვე, წელი); ლ) ჟურნალში მოსწავლეთა მშობლების (კანონიერი წარმომადგენლების) შესახებ მონაცემთა გვერდზე აღინიშნება მოსწავლის მშობლის (კანონიერი წარმომადგენლის) სახელი, გვარი და საკონტაქტო ტელეფონის ნომერი; მ) ჟურნალში შენიშვნების გვერდის შევსებაზე უფლებამოსილი პირი (კლასის დამრიგებელი, მასწავლებელი, სკოლის დირექტორი ან/და მის მიერ განსაზღვრული პირი) შენიშვნების გვერდზე მიუთითებს შენიშვნის რიგითობას, შენიშვნის შეტანის თარიღს, შენიშვნის შინაარსს და აღნიშნულს ამოწმებს თავისი ხელმოწერით. 4. საგნის ტრიმესტრული ნიშნის გამოყვანის წესი ა) საგნისათვის განსაზღვრულ თითოეულ კომპონენტში იანგარიშება საშუალო ქულა, ანუ  მოსწავლის მიერ საგნის ერთ-ერთ კომპონენტში მიღებულ ქულათა ჯამი უნდა გაიყოს ქულების რაოდენობაზე (მაგალითად, თუ მოსწავლეს ერთ-ერთ კომპონენტში ტრიმესტრის განმავლობაში აქვს 9; 8; 6; 8 და 8, მისი საშუალო ქულა იქნება (9+8+6+8+8): 5=7,8). ბ) კომპონენტის საშუალო ქულა უნდა გამრავლდეს ხვედრით წილზე (მაგალითად, თუ კომპონენტის ხვედრითი წილია 20%, მაშინ ქულათა საშუალო არითმეტიკული გამრავლდება 0,2-ზე, ანუ 7,8 X 0,2 = 1,56). გ) კომპონენტების ხვედრითი წილის გათვალისწინებით გამოყვანილი ქულები უნდა შეიკრიბოს  (მაგალითად, თუ მოსწავლეს პირველ კომპონენტში აქვს – 1,56, მეორე კომპონენტში- 2,32,  მესამე კომპონენტში – 1,77,  ხოლო მეოთხე კომპონენტში – 2,43,  საგნის წლიური ნიშანი იქნება 1,56 + 2,32 + 1,77 + 2,43 = 8.08);
